Question 1: ALT / PrtScr do not for me in Firefox 4.0.1, and neither even make sense. Overall this is a Windows reserved strike precisely the reason I mentioned. ALT and / PrtScr had this feature since Windows 95. Try disabling all add-ons you can use and see if you put a finger on them. Question 2: Click to add a new folder and select New Folder.
